The report also outlines opportunities for healthcare organizations to better support their workforces and implement cross-functional harm prevention.<\/p>\n\n\n\n<p>\u201cWe cannot begin to address the issues facing patient safety without first identifying the factors that contribute to tragic and inequitable outcomes,\u201d McDonald said.
