USA Health’s Chief Physician Executive explains how data and structure create the foundation for sustainable transformation.

The healthcare industry is consolidating rapidly. According to a 2025 Bipartisan Policy Center report, regional hospital systems are merging into larger networks, academic medical centers are acquiring community hospitals, and ambulatory practices are joining health systems at unprecedented rates. For leadership teams navigating these integrations, the technical challenges – IT systems, supply chain, billing – are straightforward compared to the cultural ones.

Dr. Michael Chang, Chief Physician Executive and System Chief Medical Officer at USA Health in Mobile, Alabama, knows this reality well. As a professor of surgery and executive leader at an academic health system experiencing growth, he’s learned that bringing organizations together requires more than messaging about shared values.

"When you bring multiple cultures together across an organization, oftentimes you have disparate or different cultures,” Dr. Chang explains. “It’s one thing to talk about it and say, ‘Hey, let’s all move in one direction together.’ But it’s better when you have tools upon which you can anchor to move in that direction together.”

The Challenge: Aligning Patient Safety Culture During Healthcare Consolidation

Healthcare consolidation brings together organizations with different histories, workflows, standards and, most critically, different approaches to patient safety and quality. Each facility has its own established practices, often developed over decades. Staff are comfortable with familiar systems. Change, even positive change, meets natural resistance.

USA Health needed to align previously independent organizations around a shared vision, and do it in a way that was measurable, sustainable and improved patient outcomes.

The solution wasn’t obvious. Most health systems default to training programs and town halls-tools that rarely create lasting change. Dr Chang’s answer centers on a principle that challenges conventional approaches toto culture change: you need both compelling data and structural tools to hardwire new behaviors.

"To get people to change, you’ve got to compel them to change. And the way to compel them to change is to show them in an objective way where they stand on the spectrum from optimal performance to suboptimal performance.”

Dr. Michael Chang
Chief Physician Executive, USA Health

The Framework: Using Strategy and Data to Drive Healthcare Culture Change

Dr. Chang approaches culture integration through a deliberate framework: vision drives strategy, which drives tactics. For USA Health, that vision is
concrete: the patient is at the center of everything.

"Whether you’re going to open a new service line or buy more pencils for the office, if you don’t have a vision, you can’t work down that chain of strategy and tactics to really know what direction you’re going in,” Dr. Chang explains.

Every decision must answer a single question: Is this what’s best for the patient?

"If the answer is yes, we do it,” he says. “If the answer is no. we don’t do it.”

That patient-centered vision leads directly to safety as the operational foundation. Everything else-quality, outcomes, patient experience-depends on preventing harm first.

Creating a Data-Driven Foundation for Patient Safety Culture

With vision established, USA Health needed the infrastructure to translate that vision into daily practice across disparate teams. This is where RLD360 emerged as a cultural anchor.

"It gives us something to center ourselves around,” Dr. Chang explains. “That centeredness is quality, safety and high reliability. If you’re trying to bring two groups of people together or two organizations together and you say, ‘Let’s all move in one direction around quality, around everything we do being centered around the patient,’ you have something to sink your teeth into when you’re implementing culture change.”

The ability to “hardwire” culture change, as Dr. Chang describes it, meant USA Health could move beyond exhortation or training alone. The organization embedded new behaviors into daily workflows, created objective measurements of performance and built accountability structures that reinforced patient-centered culture.

Measuring Healthcare Performance Through Data-Driven Reporting

Nine months before a scheduled Joint Commission visit, USA Health implemented the audits and standards capabilities offered within RLD360. The goal was to make systematic rounding and documentation standard practice across the organization.

The results spoke for themselves. USA Health logged 3,379 rounding episodes in nine months, each linking findings to policies and corrective actions. Three years earlier, the organization had received nine conditional findings from the Joint Commission. In their most recent visit, they received one.

"It may not be causal, but there’s certainly a relationship there,” Dr. Chang notes. “We use that data to show the board what the benefits could be, and it tells a very powerful story.”

The data convinced the board. Leadership needed evidence that systematic rounding was worth the investment, and the nine-to-one improvement in conditional findings provided it.

"It’s one thing to say, ‘be better’ or ‘be patient-centered,’ but it’s another thing to say, ‘We’re going to be better performers. We’re going to provide optimal care ina patient-centered way, and here’s how we’re going to do it with these tools.”

Dr. Michael Chang
Chief Physician Executive, USA Health

Driving Healthcare Culture Change Through Objective Performance Data

"Human beings by nature do not like to change,” Dr. Chang observes. “Forcing them to change typically doesn’t work in the long term, so you’ve got to compel them to change.”

Compulsion requires showing people objectively where they stand. With data, the conversation shifts from “you need to change” to “here’s where we are, here’s where we need to be, and here’s the path to get there.”

USA Health is using the platform to create a 360-degree view of organizational performance, linking governance, quality and safety and regulatory compliance. The vision is ambitious: objectively measure physician performance across clinical care, professionalism, communication and system-based practice. Instead of rubber-stamping medical staff reappointments every two years, USA Health is tying credentialing decisions to actual performance data.

Key Lessons for Healthcare Leaders Managing Cultural Integration

Dr. Chang’s experience at USA Health offers several principles for leaders managing cultural integration:

Start with a clear vision
Patient-centeredness isn’t a platitude-it’s a decision-making filter. Every choice should be testable against this criterion.

Use data to compel, not force
People resist change imposed on them but engage with change when they can see objective evidence of need and progress.

Hardwire culture through structure
Tools and processes aren’t bureaucracy-they’re the infrastructure that makes desired behaviors sustainable.

Measure what matters, not just what’s easy
Define your data needs on the front end based on what you’re trying to achieve, rather than working backward from available data.

Make the board your ally
Executive leadership needs data that demonstrates ROI on culture investments. Show them concrete results-like moving from nine conditional findings to one.

Why Cultural Integration Matters in Modern Healthcare Systems

Every healthcare M&A deal promises cultural integration. Few deliver it.

"For those people whose responsibility is to drive culture change, you can’t do it without tools,” Dr. Chang reflects. “You’re only as good as the players you have on the field, and those players have to perform well, perform within the rules, and you’ve got to be able to measure how well they’re performing.”

USA Health’s experience proves that culture change is systematic and measurable. It happens when vision meets structure, when data meets action, and when technology serves as an anchor point for transformation.

Download

When healthcare systems consolidate, technology alone won’t align cultures.
