Developed collaboratively with safeguarding, bed management, patient safety teams, and mental health partners, the solution ensures&nbsp;timely&nbsp;assessments,&nbsp;appropriate observations, and consistent monitoring throughout each patient\u2019s detention.&nbsp;<\/p>\n\n\n\n<p>The programme has strengthened governance, improved visibility of risk, and enhanced assurance for one of the Trust\u2019s most vulnerable patient groups. It&nbsp;demonstrates&nbsp;how existing systems can be adapted to deliver safer, more proactive care.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Challenge<\/h2>\n\n\n\n<p>A patient safety&nbsp;incident&nbsp;review in 2024&nbsp;identified&nbsp;opportunities to strengthen the Trust\u2019s oversight of patients detained under the Mental Health Act. This included improving the timeliness and consistency of assessments, observation practices, and visibility of patient progress.&nbsp;<\/p>\n\n\n\n<p>Patients detained under the MHA are often cared for in busy acute hospital environments, where competing clinical priorities and operational pressures could make consistent oversight more challenging. Existing processes relied on a combination of local tracking, informal communication and retrospective reporting, which limited real-time visibility.&nbsp;<\/p>\n\n\n\n<p>This made it harder to provide consistent assurance that all required actions and safeguards were being progressed in&nbsp;a timely&nbsp;way. The Trust recognised the need for a more structured and proactive approach that would support daily monitoring, improve coordination between teams, and strengthen overall governance.&nbsp;<\/p>\n\n\n\n<div style=\"padding:56.25% 0 0 0;position:relative;\"><iframe src=\"https:\/\/player.vimeo.com\/video\/1206814285?badge=0&amp;autopause=0&amp;player_id=0&amp;app_id=58479\" frameborder=\"0\" allow=\"autoplay; fullscreen; picture-in-picture; clipboard-write; encrypted-media; web-share\" referrerpolicy=\"strict-origin-when-cross-origin\" style=\"position:absolute;top:0;left:0;width:100%;height:100%;\" title=\"York and Scarborough Teaching Hospitals_TKR(Final)\"><\/iframe><\/div><script src=\"https:\/\/player.vimeo.com\/api\/player.js\"><\/script>\n\n\n\n<h2 class=\"wp-block-heading\">The Solution<\/h2>\n\n\n\n<p>The Trust developed a structured oversight process using Datix as a real-time monitoring tool, working in partnership with Bed Managers, Safeguarding and Complex Needs Teams, Patient Safety colleagues, and Tees&nbsp;Esk&nbsp;and Wear Valley NHS Foundation Trust.&nbsp;<\/p>\n\n\n\n<p>A bespoke Datix form was introduced to capture and track all MHA detentions consistently, supported by a dashboard-based monitoring system that provides visibility at each stage of a patient\u2019s journey.&nbsp;<\/p>\n\n\n\n<p>The process includes:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Immediate notification of detention, enabling early visibility for key teams&nbsp;<\/li>\n\n\n\n<li>Daily monitoring through live dashboards, tracking observations, referrals, and assessments&nbsp;<\/li>\n\n\n\n<li>Structured tracking of each patient journey, from detention through to discharge, transfer, or lifting of section&nbsp;<\/li>\n\n\n\n<li>Clear ownership and defined responsibilities, improving accountability across teams&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>Information entered by Bed Managers feeds directly into&nbsp;an initial&nbsp;dashboard, enabling rapid identification of new detentions. Updates from safeguarding and patient safety teams move cases into active monitoring, ensuring ongoing review and coordinated oversight.&nbsp;<\/p>\n\n\n\n<p>This approach has repositioned Datix as a proactive safety tool, strengthening communication, supporting consistent practice, and embedding robust governance across the Trust.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Results &amp; Next Steps<\/h2>\n\n\n\n<p>The programme has delivered measurable improvements in patient safety, governance, and organisational oversight:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>51 patients monitored in real time since implementation&nbsp;<\/li>\n\n\n\n<li>Improved timeliness of assessments, referrals, and reviews&nbsp;<\/li>\n\n\n\n<li>Increased visibility of patients detained across all wards&nbsp;<\/li>\n\n\n\n<li>Reduced reliance on informal communication and retrospective reporting&nbsp;<\/li>\n\n\n\n<li>Information from the Datix dashboard is used to inform the daily Trust operational meetings&nbsp;<\/li>\n\n\n\n<li>Supportive and reciprocal relationships established with mental health partners&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>Real-time dashboards now provide assurance that each patient\u2019s care is actively monitored, enabling earlier intervention where required and reducing delays.&nbsp;<\/p>\n\n\n\n<p>The data generated has also supported wider service improvement. Analysis has identified patterns in detention activity, enabling more targeted workforce planning, staff training, and escalation responses.
