The key themes were:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>National strategies are written as &#8220;work as imagined.&#8221;, while frontline staff need them translated into what is feasible on the ground\u00a0<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Good care events are mandated through LFPSE but are still reported far less than incidents. Organisations that defined what good care looks like and kept forms simple reported significant increases in reporting\u00a0<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Psychological safety remains a barrier to cultural change. Staff at one ambulance service now trust Datix enough to report sensitive incidents, requiring new access controls to protect sensitive records\u00a0<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Quality data is only useful if frontline staff believe it is accurate. One trust had to do significant work with BI to earn that credibility\u00a0<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The phrase &#8220;I&#8217;m going to Datix you&#8221; is still heard in trusts and signals that the culture shift from blame to learning is not complete\u00a0<\/li>\n<\/ul>\n\n\n\n<div style=\"height:25px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 id=\"h-what-organisations-shared-about-quality-improvement-and-good-care\" class=\"wp-block-heading\"><strong>What organisations shared about quality improvement and good care<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Multiple organisations described practical approaches. A matron from University Hospitals North Midlands explained how paper run charts on ward boards, populated weekly from Datix dashboards, make quality data accessible to staff nurses and healthcare assistants. Northwest Ambulance Service described a quality improvement network that includes non-clinical teams like finance, and a violence and aggression project that used QI methodology and introduced body-worn video cameras.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On good care, RUH Bath has added a question to the incident form asking whether there were examples of good care during the event. South East Coast Ambulance previously ran a standalone good care form with five or six questions and reported a significant increase in submissions after defining what good care looks like. Inspire North has adopted after-action reviews for positives as well as incidents, arguing that if staff experience the process as a celebration tool, they will embrace it when it is used for learning.<\/p>\n\n\n\n<div style=\"height:25px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 id=\"h-what-this-means-for-organisations-and-who-it-is-relevant-to\" class=\"wp-block-heading\"><strong>What this means for organisations and who it is relevant to<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The discussion surfaced a tension between what strategies ask of organisations and what is feasible on the ground. A chief nurse described two gaps that boards cannot currently fill: real-time patient experience data across pathways and real-time staff experience data that goes beyond red flags to capture what matters to staff as people. The discussion highlighted the need for better collection and use of this information to support genuine quality assurance.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On board engagement, several organisations described assigning board members to specialties and linking risks directly to named directors, with committee structures covering people and culture, operations, and audit and risk separately before bringing everything together. Northwest Ambulance Service described investigating incidents from the patient&#8217;s perspective, from call to hospital door, rather than from the perspective of whichever service happened to be involved.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hannah Dalziel closed by observing that quality is already at the forefront of everything the room is doing, even before the strategy is published, and challenged the room to be kinder to themselves about how far they have come in two years.
