By standardising and integrating safety and performance data, directors and care group leads now have a unified, dependable view of trends and risks. This accessible and consistent intelligence enables timely decisions, targeted interventions, and improved strategic planning. The approach supports higher quality care, strengthens accountability, and builds greater confidence in the organisation\u2019s data.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>The Challenge<\/strong>&nbsp;<\/h2>\n\n\n\n<p>Reporting across care groups was fragmented, with inconsistent figures and varied standards limiting visibility of performance against director-set priorities. Directors and leaders often had to navigate decisions with incomplete or delayed information, which made it harder to maintain clarity and alignment. Disparate data sources, a lack of comparability, and delays in accessing meaningful analysis reduced the ability to identify trends or respond promptly to emerging risks\u2014creating challenges for strategic oversight and shared accountability.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>The Solution<\/strong>&nbsp;<\/h2>\n\n\n\n<p>The Patient Safety Team led the initiative in partnership with Trust Directors and Care Group Leads. Together they:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mapped existing data flows and defined essential safety indicators&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Aligned reporting standards with director-set KPIs&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Deployed Yellowfin dashboards to provide real-time, interactive views of key metrics such as risk reviews, complaint timeliness, and overdue actions&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>Directors shaped priorities, care group leads validated metrics, and the collaborative approach ensured the information was both relevant and accurate. Dynamic dashboards replaced manual exports, enabling instant visualisation of performance, risk, and safety data.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Results &amp; Next Steps<\/strong>&nbsp;<\/h2>\n\n\n\n<p>In the past 18 months the project has delivered clear, measurable improvements:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Average overdue complaints reduced from 17 cases (14.62%) to 12 cases (11.86%)&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>3 hours saved per team (5 teams) in preparing performance assurance reports, with further rollout planned to specialty level reporting&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>22% improvement in risks reviewed on time&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>Dashboards are now in active use across all care groups, forming part of routine governance and performance reviews. Directors and teams can interrogate live data directly, embedding a culture of transparency and continuous improvement.
