LFPSE Hub
Patient Safety Incident Response Framework in DCIQ and DatixWeb
Background Insight
What is LFPSE
LFPSE (Learn from Patient Safety Events) is replacing the current NRLS (National Reporting and Learning System). We’re here to support you with this transition, whether you have gone live or are going live.
On this page you can find everything you need to implement LFPSE in DCIQ or DatixWeb.
LFPSE Taxonomy V6
We are pleased to announce that LFPSE V6 is now available in DatixWeb and will soon be available in DCIQ.
NHS England Resources
NHS England have created a large amount of resources to help support you through the implementation and roll out of LFPSE. We have linked the key resources, and will continue to update the links as more are created.

The Journey to LFPSE
We are proud to say that both DCIQ and DatixWeb are fully compliant and approved for LFPSE use by NHS England. However, we recognize that compliance is only the first step in the journey to LFPSE.

LFPSE Customer Resource Hub
We are dedicated to ensuring that you have everything you need to successfully go live with LFPSE and then ensure the system is integrated into your service. To that end, we have created a range of resources, and will continue to update and make more tools to help you achieve this.
You can view all of our LFPSE resources directly via our Customer Support Portal, Zendesk, which you can access via the links below.
DCIQ and DatixWeb Ideas Portal
Customer feedback is the key driving factor to help us decide what new features and upgrades we develop for both DCIQ and DatixWeb.
Our DCIQ and DatixWeb Ideas Portal is one of the key ways we gather this feedback directly from our users on what features and improvements they would like to see.
On the Ideas Portal you can directly feedback your own DCIQ and DatixWeb ideas, and vote on other customers ideas to help us . Our product team will then analyse the results and top voted for ideas, to help plan product roadmaps.
If you have an idea for a new feature for LFPSE, or any other part of DCIQ or DatixWeb, put your idea forward on the Ideas Portal.
Got more questions?
Yes. Organisations using DatixWeb or DCIQ can report to LFPSE through their RLDatix system. Both are approved and compliant for LFPSE reporting by NHS England.
The NHS England online LFPSE service is a direct reporting route for providers without a local risk management system. It is not a replacement for DatixWeb or DCIQ users.
Contact RLDatix support via the Customer Support Portal if you need help.
Yes. DatixWeb and DCIQ are fully compliant and approved for LFPSE reporting by NHS England. Organisations using either system have a confirmed reporting route and do not need to switch systems.
For setup and configuration detail, see the dedicated DatixWeb LFPSE FAQs and DCIQ LFPSE FAQs.
LFPSE, or Learn from Patient Safety Events, is the national NHS service for recording and learning from patient safety events in healthcare.
Events are reported through a compliant local risk management system such as DatixWeb or DCIQ, which submits data to LFPSE automatically, supporting national safety improvement across the NHS.
Yes. LFPSE has replaced the National Reporting and Learning System (NRLS), which was decommissioned on 30 June 2024.
Organisations now record patient safety events through LFPSE, either via a compliant local risk management system such as DatixWeb or DCIQ, or through the NHS England online service where no local system is in place.
The LFPSE V6 taxonomy is the current reporting framework used to categorise and submit patient safety event data to NHS England. It introduces updated data categories, improved reporting consistency and additional fields supporting PSIRF.
The V6 taxonomy is available for DatixWeb users and will soon be available for all DCIQ user as well. DCIQ is still fully compliant on the V5 taxonomy. See the dedicated product FAQ pages or contact RLDatix support for detail.
